Information you choose to share

If you call, email or fill out our new client inquiry form, we receive the information you share, like your name, phone number, email address and your questions.

If your child becomes a client, our Notice of Privacy Practices explains how we use and protect your child’s health information.

Contact form

If you fill out our new client inquiry form, your answers are collected with Google Forms and stored in our Google Workspace account (Google Forms and Google Sheets), which is covered by a HIPAA Business Associate Agreement with Google. We use your responses to follow up on your inquiry. We ask only for your name, phone, email, your child’s age, your insurance company, your preferred contact method and an optional note. Please don’t include medical or diagnosis details or insurance member ID numbers. To open the form, select the “Fill out our new client inquiry form” button on our website. The form opens in a new tab on Google’s website, where Google’s own privacy policy and cookies apply.

Information collected automatically

We load fonts from Google Fonts, so Google receives your IP address and browser information when a page loads.

Our website host may keep standard server logs (such as IP address, browser type and pages visited) to keep the site running and secure.

This site uses no analytics, ads or tracking cookies. Google Fonts is the only third-party service that loads on our pages.

Do Not Track

Our website does not respond to Do Not Track signals, but it uses no cross-site tracking: no analytics, ads or tracking cookies.

Changes

If we change this policy, we’ll post the new version here with a new effective date.
